for screening, we can arrange your procedure appointment without an office
visit. If you prefer to be seen in consultation before the procedure, we
will be happy to schedule this appointment.
Call our office at (239) 939-9939 and leave a message. One of our schedulers will contact you to complete the process, set up a
time and date and obtain a medical history and detailed insurance
information by phone. Please have your insurance cards and a list of
your medications available so you can provide
complete, accurate information. We routinely request preauthorization from
your insurance carrier in order to avoid payment problems.
